Membership questions

How membership works, what it costs to start, and what is included.

What does the membership fee cover?

The fee covers excellent primary care through unlimited office visits, annual wellness exams, well-child exams, sports physicals, school physicals, basic office procedures, treatment of acute illness or minor injuries, and direct access to your doctor and our staff — all with no wait times in the office.

Members also have access to discounted labs, imaging tests, and special procedures that are not included in the membership fee.

Is there a sign-up fee to join?

Yes. There is a sign-up fee of $75 per person, $150 maximum per enrollment session. If you signed up a family of five on five different dates, each person would pay $75. Sign up all five on the same day and $150 is the most you would pay.

If you cancel and rejoin, you will be asked to pay the initiation fee again. Employer groups of 5 or more do not pay initiation fees.

Does Elevated Health see patients of all ages?

Yes. Elevated Health currently provides care for patients of all ages.

What if I decide to cancel?

We expect an open, honest, respectful relationship with you. There is a minimum commitment of 90 days, and we anticipate a long-term patient–physician relationship. Please give 30 days’ notice if you wish to cancel. If you choose to rejoin, you may be charged a re-initiation fee.

How are medical costs managed in your DPC practice?

Our practice offers labs, medication, and imaging at costs that are usually lower than traditional insurance copays and deductibles. If you find lower costs elsewhere, our providers can send orders so you can use your insurance there.

How does your practice help me avoid hitting my deductibles?

Membership covers many medical procedures, exams, and tests without referrals or trips to the ER or urgent care. By addressing needs proactively we aim to minimize unnecessary costs and help you avoid reaching your deductibles.

Do I need to worry about surprise bills or copays in your DPC model?

No. You will not encounter surprise bills, balance bills, or copays for membership care. Pricing is transparent and predictable.

Is the membership suitable for individuals of all ages?

Yes. The program is an option for children through seniors. Care is tailored to each patient’s age and health status.

How do I get started with Elevated Health?

What Direct Primary Care is, how visits work, and how we handle records and travel.

What is Direct Primary Care?

Direct Primary Care is a healthcare model that eliminates copays and deductibles for primary care, with shorter waits and same-day visits. You have direct access to your doctor without those financial barriers.

Under the DPC model you receive comprehensive primary care directly from your physician. You pay a monthly fee directly to Elevated Health.

Because DPC practices do not participate in insurance, providers avoid the documentation demands, administrative intrusions, and patient-data collection that insurance plans often impose. Those requirements cost staff, time, and money, and they often intrude on privacy.

Without those obligations, physicians can spend that time listening to and treating patients — and pass the overhead savings on to you.

How does DPC benefit the doctor–patient relationship?

No insurance company or pharmacy stands between you and your doctor. Care is tailored to your needs rather than dictated by external factors.

Do you see nursing home patients?

Yes. Our physicians see members who are nursing home patients.

What medical conditions do you care for?

We treat a wide array of acute and chronic health problems in pediatric, adult, and geriatric patients.

What if I am hospitalized?

Your Elevated Health physician will work with the doctors who see you if you need to be admitted, and stay involved so you get back to health as quickly as possible.

Are my medical records ever shared with insurance carriers or government agencies?

No. Patient privacy is a natural benefit of this model. We will never give a third party a copy of your records unless you specifically ask us to, or Elevated Health is subject to a subpoena or search warrant.

What if I need medical attention while I’m away from home?

Many conditions can be diagnosed and treated by phone or video. If appropriate, we will locate the nearest pharmacy and order medication, or mail medications to you. If you need a hospital or other care while away, we can advise you on that as well.

May I contact Elevated Health after hours?

Yes. After office hours, members can still call and text their physician.

When do I pay my fees for non-covered labs, imaging, and procedures?

Charges are added to your preferred method of payment. We will always tell you what you are being charged for before you are billed.

Insurance

Medicare and insurance questions

How membership works with insurance, Medicare, and health-sharing plans.

Do I still need insurance?

We do not require insurance for membership. We recommend that you do have coverage to comply with federal law. Elevated Health does not replace insurance. Ideally, members carry a high-deductible or major medical plan, or a plan with a health savings account, so they have help if they are hospitalized or need a specialist.

Consult your insurance provider to find a plan that works with this model. We also care for patients in health-sharing ministries such as Liberty HealthShare and Medi-Share.

What if I want to continue my current insurance? Can I use it?

Not for membership visits. We will work with your insurance to submit labs, X-rays, and medications we prescribe. If your plan requires a particular facility, tell us.

If I have Medicare, can I join as a patient?

Yes. Your monthly membership fee cannot be submitted to Medicare for reimbursement. You must sign a waiver every two years that neither you nor your doctor will bill Medicare for the membership fee.

Medicare and Medicare supplemental insurance still cover medical costs outside Elevated Health — for example prescriptions at an outside pharmacy, labs run at an outside lab, hospital charges, physical therapy, X-rays, and scans. You do not lose Medicare coverage for other needs by joining.

How can I save on insurance costs by being a patient of Elevated Health?

Many people buy expensive “Cadillac” plans that require little or no copay and cover labs and prescriptions. With Elevated Health, the monthly fee covers office visits with no copays, and members have access to wholesale medications and labs — sometimes at a 90% discount. It can be worthwhile to change from a Cadillac plan to a basic plan and still get better primary care.

Are Elevated Health’s monthly fees eligible for HSA or FSA reimbursement?

Many patients find that DPC fees are reimbursable through an HSA or FSA. Consult your tax advisor for your circumstances. See the HSA questions below for the federal rule that takes effect January 1, 2026.

HSA and Direct Primary Care

Starting January 1, 2026, qualified DPC memberships can be paid with HSA funds.

Can I use my HSA to pay for Direct Primary Care?

Using an HSA for a DPC membership was long a gray area. Starting January 1, 2026, federal law allows it for HSA plans nationwide. You will be able to pay your DPC membership with pre-tax HSA funds, no matter which bank or HSA administrator you use.

When does the new HSA law take effect?

January 1, 2026. After that date, HSA funds can be used for DPC memberships without affecting your eligibility to contribute to your HSA.

How much of my HSA can I use for DPC?

You will be able to use up to:

  • $150 per month for an individual membership
  • $300 per month if your plan includes a spouse or dependents

This applies to qualified Direct Primary Care memberships, including Elevated Health.

How does paying with my HSA help me save money?

Your HSA uses pre-tax dollars, which can save about 20–30% on the monthly DPC fee, depending on your tax bracket.

Can I still keep my insurance plan if I join a DPC and pay with an HSA?

Yes. Most patients pair membership with a high-deductible health plan (HDHP) to cover emergencies or hospitalizations. DPC + HDHP + HSA is one of the more cost-effective ways to stay covered.

How do I start using my HSA for my DPC membership?

If you already have an HSA, contact us to set up recurring DPC payments. If you are new to HSAs, our team can help you through setup.
